If you’re looking for the Sword of Milos in Elden Ring, you’ll need to get familiar with the red NPC at the back of the Roundtable Hold. He is called “Dung Eater” in the game, as he’s surrounded by corpses, which is probably from his doing.
You can interact with this NPC and speak to him, but he won’t progress the quest without the correct items.

To earn the Sword of Milos in Elden Ring, you will need a Seedbed Curse to progress the Dung Eater’s quest. You can find one by heading down the first elevator in Leyndell to a room with a perfumer.
From there, take the ladder and follow the path to locate the item. Bring back to Seedbed Curse to the Dung Eater, and he’ll give you a sewer key.
Once done, take the key down to the Subterranean Shunning Grounds under Leyndell.
Follow the sewer system until you reach an open area closed off by gates. On the other side of the gate is the true form of the Dung Eater. Use the sewer key to let the form go, or you can kill it with brute force.

Defeating the Dung Eater rewards you with the Omen armor and the Sword of Milos.
